Rehab At Home is just what it sounds like: outpatient physical therapy provided in the comfort and convenience of your own home. We strive to produce immediate results for increased functionality by focusing on relevant and personalized instruction.
We work primarily in Steuben and Chemung County, specializing in geriatrics and certified case management. Our patients are not home bound by Medicare Part A standards, but may have difficulty with transportation.
Rehab At Home is covered by Medicare B and most private insurance companies.

Geriatric Care Management
We provide comprehensive in-home assessments to determine immediate needs and develop personalized goals. Our rehabilitation background focuses on maximizing independence and the safety of our patients, while enhancing quality of life. We work with family members and caregivers as a patient advocate in a wholistic approach to care management.
Our services are personalized based on the in-home assessment. Offering family/ caregiver support and problem solving, regular monitoring, medication review and management, bill management, residential options, crisis support, alternative residency options, appointments, and more. We strive to share helpful resources and practical information with patients and caregivers.
Phone consultations are free and we accept private pay.
